Ten-year-old computer glitch prevents delivery of 1,380 Canadian health results

Sunday, May 24, 2009

The Saskatoon Health Region in Saskatchewan, Canada recently discovered a fax machine problem which had not relayed almost 1,500 X-rays, Computed tomography (CT) and Magnetic resonance imaging (MRI) scans, ultrasounds, and other medical imaging test results to doctors.

The fax machine is part of the automated Radiology Information System. An audit revealed that medical diagnostic tests involving results dating back a decade have been affected by the broken fax machine.

On May 6, a doctor called the health region officials seeking follow up on a patient’s report. Staff who delved into it, found that the report was not sent from the system.

Now, two weeks later, this same patient has still not been contacted by the health region.

“It has taken some time to work with the patient’s physician. That was important — to get more information and more detail because we want that patient’s physician to be involved in the discussion with the patient,” Dr. David Poulin, vice-president of medical affairs for The Saskatoon Health Region, said. “This is a system error and that’s just what it was — an error. This doesn’t reflect in any way on the quality of work regional staff have done and continue to do.”

An internal review revealed that, of 2.2 million diagnostic tests performed, at least 1,380 had not been sent out by the malfunctioning fax machine.

The health region will embark on contacting each of the 1,380 patients and their physicians to make sure that the results have been received and if any health care was compromised.

Patients can also contact the health region via a newly set up hotline to make enquiries.

“We think it would actually be good practice if physician offices could have a system to check whether they have received important reports,” said Poulin, “The common practice appears to be in many doctors’ offices, particularly family physician offices, that they don’t respond to the report until it arrives. So, they basically are waiting for the report to arrive in their office by fax and it’s at that point that they look at it and decide what action to take.”

City Hospital, St. Paul’s Hospital and Royal University Hospital were the three city hospitals affected. Saskatoon has a population of roughly 233,923, and the hospitals also serve the surrounding rural areas.

Ironically the faulty fax machine was discovered the same week that provincial medical officials began to review approximately 70,000 radiology tests conducted in Yorkton. Officials there doubt the competence of the physician who first interpreted the radiology results.

Iran stands firmly against nuclear agency resolution

Friday, August 12, 2005

In a nationally broadcast radio sermon by former Iranian president Akbar Hashemi Rafsanjani on Friday, he said of the U. N. watchdog group adopting the resolution for Iran to stop nuclear enrichment activities, “The decision was a cruel one.”

The Thurday adoption in Vienna by the governing board of the International Atomic Energy Agency (IAEA) was unanimous and legally non-binding in asking Iran to suspend all nuclear fuel related activities. The board also requested a report from the agency’s chief, Mohamed ElBaradei, on its compliance by Tehran.

On Thursday, an Iranian member of their nuclear negotiating team, Sirous Nasseri, read in London a statement made by his republic at the emergency governing board meeting of the IAEA regarding the Nuclear proliferation treaty (NPT):

How can this body be called to react to an act, which is in full conformity with the NPT and the safeguards and constitutes a limited manifestation of the exercise of an inalienable right? A right, which by its own simple meaning, cannot be alienated from anyone.

IAEA spokeswoman Melissa Fleming, said yesterday, “The resolution on Iran was just adopted without a vote, by consensus. So, full consensus. All 35 members of the board agreed to the language of the resolution text.”

International concern over the Iranian nuclear enrichment program was led by the European Union countries of France, Great Britain, and Germany (EU-3), along with the United States. The pressure brought by these nations succeeded in obtaining language calling for the re-instatement of seals that were removed at the Uranium Conversion Facility in Isfahan. The seals were removed during the recent installation of surveillance cameras by IAEA.

The agency is to monitor Tehran for compliance to halt uranium enrichment. After a September 3 deadline, Iran again faces a possible referral to the U.N. Security Council where it could be subject to economic sanctions.

Meanwhile, the United States is likely to grant an entry visa to Iranian President Mahmoud Ahmadinejad to attend the United Nations opening session next month in New York City, New York.

The Bush administration probe into whether Ahmadinejad was connected to the 1979-1981 Iranian hostage crisis is looking like it was a case of mistaken identity. The decision remains tentative as intelligence reports within the administration continue to circulate, according to The Washington Post citing U.S. officials.

How To Play Casino Games

Far away is not always a beginner, and even a fairly experienced player in an online casino adheres to certain rules, or if you like, a code of conduct. We advise as soon as possible to develop our own principles, which must be followed even before the start of the game and on time. Perhaps some recommendations on how to play in online casinos, given by us, will seem useful to those of our readers who are just beginning to get acquainted with the fascinating and exciting world of online casinos.

Best tips for online casino players

Always thoroughly study the rules of the game before it starts. If you seriously intend to reduce the advantage of the casino, knowledge of the rules and understanding of the principles of the game are imperative. To know how to play online casinos, you need to listen to our advice. Do not strive to make money in a casino, this is a rather pointless exercise. Constantly winning is impossible, you will always be behind the institution. At the same time, we do not urge you to play attentively at all, on the contrary. A well-conducted game session often ends in a win. But in the first place should always be the pleasure of the game.

Do not forget to use bonus programs. Even if not all of them are interesting or profitable, however, nevertheless, with their help you can sometimes take part in some interesting tournaments or win a valuable prize, mentioned on the web site. Carefully choose a casino worthy of your starting a cash online gambling game. Study forums, ratings, thematic sites. Do not rush to the first interesting offer.

Never, under any circumstances, try to fool casinos online or other players. With a very high degree of probability, you will be caught by the hand, and the best option for the development of such a situation would be to block your account and add your personal data to the blacklist. If you suspect that you yourself have become a victim of foul play, contact the administration of the institution or the regulatory authorities. Their list is presented on the online casino website.

When playing online blackjack, baccarat, roulette, or video poker, do not forget to use mathematically correct strategies. Only with their use, you will be able to reduce the advantage of the institution to a fairly low level. Do not rely on intuition when confronted with a random number generator. Before starting the game in an online casino, determine in advance the maximum acceptable amount of loss, and do not exceed this value. Also never stay at the gaming table beyond the scheduled time.

Never pay the debt in the casino. The consequences of such a game can be very unpleasant. Perhaps this is the most dangerous thing in the casino, which can trap an unlucky player. We hope some of our tips on how to play online casinos will be useful not only for beginners but also for experienced users of online casinos.

Saturn moon Enceladus may have salty ocean

Thursday, June 23, 2011

NASA’s Cassini–Huygens spacecraft has discovered evidence for a large-scale saltwater reservoir beneath the icy crust of Saturn’s moon Enceladus. The data came from the spacecraft’s direct analysis of salt-rich ice grains close to the jets ejected from the moon. The study has been published in this week’s edition of the journal Nature.

Data from Cassini’s cosmic dust analyzer show the grains expelled from fissures, known as tiger stripes, are relatively small and usually low in salt far away from the moon. Closer to the moon’s surface, Cassini found that relatively large grains rich with sodium and potassium dominate the plumes. The salt-rich particles have an “ocean-like” composition and indicate that most, if not all, of the expelled ice and water vapor comes from the evaporation of liquid salt-water. When water freezes, the salt is squeezed out, leaving pure water ice behind.

Cassini’s ultraviolet imaging spectrograph also recently obtained complementary results that support the presence of a subsurface ocean. A team of Cassini researchers led by Candice Hansen of the Planetary Science Institute in Tucson, Arizona, measured gas shooting out of distinct jets originating in the moon’s south polar region at five to eight times the speed of sound, several times faster than previously measured. These observations of distinct jets, from a 2010 flyby, are consistent with results showing a difference in composition of ice grains close to the moon’s surface and those that made it out to the E ring, the outermost ring that gets its material primarily from Enceladean jets. If the plumes emanated from ice, they should have very little salt in them.

“There currently is no plausible way to produce a steady outflow of salt-rich grains from solid ice across all the tiger stripes other than salt water under Enceladus’s icy surface,” said Frank Postberg, a Cassini team scientist at the University of Heidelberg in Germany.

The data suggests a layer of water between the moon’s rocky core and its icy mantle, possibly as deep as about 50 miles (80 kilometers) beneath the surface. As this water washes against the rocks, it dissolves salt compounds and rises through fractures in the overlying ice to form reserves nearer the surface. If the outermost layer cracks open, the decrease in pressure from these reserves to space causes a plume to shoot out. Roughly 400 pounds (200 kilograms) of water vapor is lost every second in the plumes, with smaller amounts being lost as ice grains. The team calculates the water reserves must have large evaporating surfaces, or they would freeze easily and stop the plumes.

“We imagine that between the ice and the ice core there is an ocean of depth and this is somehow connected to the surface reservoir,” added Postberg.

The Cassini mission discovered Enceladus’ water-vapor and ice jets in 2005. In 2009, scientists working with the cosmic dust analyzer examined some sodium salts found in ice grains of Saturn’s E ring but the link to subsurface salt water was not definitive. The new paper analyzes three Enceladus flybys in 2008 and 2009 with the same instrument, focusing on the composition of freshly ejected plume grains. In 2008, Cassini discovered a high “density of volatile gases, water vapor, carbon dioxide and carbon monoxide, as well as organic materials, some 20 times denser than expected” in geysers erupting from the moon. The icy particles hit the detector target at speeds between 15,000 and 39,000 MPH (23,000 and 63,000 KPH), vaporizing instantly. Electrical fields inside the cosmic dust analyzer separated the various constituents of the impact cloud.

“Enceladus has got warmth, water and organic chemicals, some of the essential building blocks needed for life,” said Dennis Matson in 2008, Cassini project scientist at NASA’s Jet Propulsion Laboratory in Pasadena, California.

“This finding is a crucial new piece of evidence showing that environmental conditions favorable to the emergence of life can be sustained on icy bodies orbiting gas giant planets,” said Nicolas Altobelli, the European Space Agency’s project scientist for Cassini.

“If there is water in such an unexpected place, it leaves possibility for the rest of the universe,” said Postberg.

The Tester victory by less than 3,000 votes was projected at approximately 2 p.m. EST after the State of Montana announced the results of overnight recounts. Democrat Jim Webb has prevailed in that race by slightly more than 7,000 votes, though his opponent has not conceded and a recount may still occur.

Wikinews interviews John Wolfe, Democratic Party presidential challenger to Barack Obama

Sunday, May 20, 2012

U.S. Democratic Party presidential candidate John Wolfe, Jr. of Tennessee took some time to answer a few questions from Wikinews reporter William S. Saturn.

Wolfe, an attorney based out of Chattanooga, announced his intentions last year to challenge President Barack Obama in the Democratic Party presidential primaries. So far, he has appeared on the primary ballots in New Hampshire, Missouri, and Louisiana. In Louisiana, he had his strongest showing, winning 12 percent overall with over 15 percent in some congressional districts, qualifying him for Democratic National Convention delegates. However, because certain paperwork had not been filed, the party stripped Wolfe of the delegates. Wolfe says he will sue the party to receive them.

Wolfe will compete for additional delegates at the May 22 Arkansas primary and the May 29 Texas primary. He is the only challenger to Obama in Arkansas, where a May 10 Hendrix College poll of Democrats shows him with 38 percent support, just short of the 45 percent for Obama. Such an outing would top the margin of Texas prison inmate Keith Russell Judd, who finished 18 percent behind Obama with 41 percent in the West Virginia Democratic primary; the strongest showing yet against the incumbent president. Despite these prospects, the Democratic Party of Arkansas has already announced that if Wolfe wins any delegates in their primary, again, due to paperwork, the delegates will not be awarded. Wolfe will appear on the Texas ballot alongside Obama, activist Bob Ely, and historian Darcy Richardson, who ended his campaign last month.

Wolfe has previously run for U.S. Congress as the Democratic Party’s nominee. On his campaign website, he cites the influence “of the Pentagon, Wall Street, and corporations” on the Obama administration as a reason for his challenge, believing these negatively affect “loyal Americans, taxpayers and small businesses.” Wolfe calls for the usage of anti-trust laws to break up large banks, higher taxes on Wall Street, the creation of an “alternative federal reserve” to assist community banks, and the implementation of a single-payer health care system.

With Wikinews, Wolfe discusses his campaign, the presidency of Barack Obama, corporations, energy, the federal budget, immigration, and the nuclear situation in Iran among other issues.

Teen broadcasts suicide online

Sunday, November 23, 2008

A Pembroke Pines, Florida teenager killed himself Wednesday, November 19, while broadcasting on the live video site Justin.tv. After making suicide threats and being encouraged by Justin.tv viewers and Bodybuilding.com forum members, Abraham K. Biggs, 19, committed suicide by taking an overdose of opiates and benzodiazepine, which had been prescribed for his bipolar disorder.

Biggs first began blogging about his planned suicide 12 hours before the actual event. He died after taking pills and lying on the bed in front of the webcam. After the broadcast, viewers who apparently thought it was a hoax posted messages such as “OMG”, “LOL”, and “hahahah”.

Hours later, after being alerted by viewers who had noticed that Biggs had stopped breathing, law enforcement and paramedics arrived, discovered his body, and covered the camera. The Broward County Medical Examiner’s Office has reportedly confirmed Biggs’ death.

According to Montana Miller of the Bowling Green State University, the circumstances of this case were not shocking: “If it’s not recorded or documented, then it doesn’t even seem worthwhile. For today’s generation it might seem, ‘What’s the point of doing it if everyone isn’t going to see it?'”

Biggs’ sister Rosalind was angry that neither the website nor its viewers reacted soon enough to save him. “They got hits, they got viewers, nothing happened for hours,” she said. She described him as “very happy” and “friendly and outgoing.” “On a normal day, you couldn’t really tell that he got as low as he did.” However, he did have relationship problems with his girlfriend, according to a friend.

Mental health professionals have warned about the possibility that other mentally troubled people would copy his actions. According to Dr. David Shaffer of Columbia University, “Any video showing it as heroic or romantic or glamorous could reduce the anxiety people might feel about suicide. It becomes a respectable behavior and lowers the threshold of suicide.” He and other psychiatrists recommend that potentially suicidal teens talk to others and “tell what’s going on.”
